Overview HB 166 section 3313. 6024 requires Ohio school districts report on each building, each grade kindergarten through 12, on the types of prevention-focused programs, services and supports used to assist students in: ✓ Developing knowledge and skills to engage in healthy behaviors and decisionmaking; ✓ Increasing awareness of dangers and consequences of risky behaviors: substance abuse, suicide, bullying and other harmful behaviors. Educational service centers are also required to report on prevention-focused programs, services and support they provide to their students, families and community through the Prevention Services Survey. The survey will be focused on any direct instruction you provide to students where your ESC serves as the learning environment in lieu of or in addition to a local district. For example, the ESC may provide instruction to special education students or be a learning environment for students in Alternative Education programs.

Overview Continued Educational service centers that do not provide instructional services directly have the option to answer “no” to the first question in the survey. The first question asks, “Does your ESC provide instruction to students and serve as the learning environment? ”. After answering “no” to the first question, please hit submit and your survey will be complete. For ESC’s that provide direct instruction and serves as the learning environment for students, please submit your ESC Prevention Services Survey by close of business on February 12 th, 2021. The results from the survey will be used by the Department to inform prevention policy, funding and support decisions. ESCs can use the data to identify assets and gaps in prevention services and supports and align prevention programs with student’s whole child needs. Where to Begin Each Educational Service Center (ESC) Superintendent will receive an email with a survey link specific to your ESC. If you cannot locate your link, please email support@ohio-k 12. help. Superintendents will receive an email with a secure survey link to access the ESC page. From the ESC page each ESC can submit their own survey report. The survey link should be forwarded to the professional staff in the building that has knowledge of the prevention-focused programs, services and supports provided to students receiving instruction at the ESC. Deadline to submit is close of business on February 12 th, 2021.

One PDF contains answers to the data collection for your ESC. The other PDF contains answers to data collection for each ESC. • Please save the confirmation and PDFs for your records. You will not be able to access the PDFs after the reporting window has closed.
