Well. Street Philosophy and Goal Well. Street believes that all people have a right to safety, security, and the opportunity for advancement. We believe that homelessness is not a choice that people make willingly. Our goal is to promote wellness for people on the streets through prevention, access and support.

Well. Street – A Needed Service in our county. � There approximately 66, 000

Well. Street – A Needed Service in our county. � There approximately 66, 000 homeless individuals in Michigan. � Nearly 40, 000 of these people are unsheltered with the remaining 26, 000 individuals having shelter and little else. ◦ Well. Street specifically targets the Atrium, Benzie, Grand Traverse, Kalkaska, and Leelanau counties. Here in these counties, there approximately 600 people without proper shelter. ◦ Statistics show that many of these individuals are between the ages of 14 -17 years old. By themselves, these individuals are too young to stay in the local homeless shelters.

To provide HIV, Hepatitis C, TB, Chlamydia, and Gonorrhea screening to 20 % of identified local homeless population 3. To provide treatment and appropriate follow-up for all diseases identified in objective #2. 4. Provide vision/hearing testing to 20 % of local homeless population and provide follow-up for all with identified vision/hearing problems. To deliver basic mental health and/or substance abuse treatment referral to 20% of identified homeless population. 5. 6. To provide blood pressure/blood glucose checks to 20% of identified homeless population 7. To distribute 100 food/hygiene bags monthly as part of street outreach team activities. 8.
