Signatures 1. Show your PGP form to your evaluator OR a certificated colleague, and describe or display your evidence. • • Evaluator Certificated colleague (SLP, teacher, other ESA) 2. Sign and date the PGP form. 3. Put the PGP form in a “safe place. ” You do not need to share the form with anyone else. Save it in case you are audited by OSPI.

PGP: ethics Falsifying or misrepresenting a PGP is considered an act of unprofessional conduct, and may result in losing your ESA certificate. - I declare under penalty of perjury under the laws of the State of Washington that I have completed the professional growth plan and submitted evidence to that effect. The intentional misrepresentation of a material fact in this form subjects the certificate holder to revocation of his/her certificate pursuant to chapter 181 -86 WAC.

Get clock hours! 1. Complete the Annual PGP verification. 2. Submit the verification to HR. 3. Get 30 clock hours for free. 4. Have a party.
