Why Origami Risk? • The University is committed to maintaining a safe environment that will encourage and enhance the experiences of each person who steps onto one of our campuses • An important part of improving safety is quickly identifying and responding to incidents or unsafe conditions • A new online incident reporting system, Origami Risk, has been designed to make incident reporting easy and simple for all employees and students

Effective 27 March 2015, the University of Alaska will use the on-line Origami Risk

Effective 27 March 2015, the University of Alaska will use the on-line Origami Risk program for reporting incidents and unsafe conditions. Reporting options include: • Employee incidents with/without medical treatment • Reporting an injured person (including students) • Reporting an Unsafe Condition • Reporting damaged UA and non-UA Property • Reporting damaged UA and non-UA vehicles PLEASE NOTE: All previous UA and UAF Incident/Accident reporting forms are obsolete and should not be used

Origami Risk - Benefits of this new online reporting system include: � Immediate EHSRM/UA Claims notification of the incident � Faster investigation and response process � Less resources (paperless system, no postage, no mail delays, etc. ) � Timely reports can be submitted from all systems (PC and Mac computers, tablets, smart phones, etc. ) � Better customer service for worker’s compensation and general insurance claims � Fewer forms to complete for accidents with injuries (Previously, three forms were required; now only one!)

Log-in using your UA credentials (normal username and password)

Congratulations, you may now complete the report you selected! • Please complete the form to the best of your ability • When finished, hit “Complete Incident” and the report will immediately go to EHSRM/Claims • You are no longer required to fill out the Employee and Employer Report of Occupational Illness or Injury (worker’s compensation) forms when injuries/illness require medical treatment. Origami does this automatically! • If you have any problems completing this form, contact one of the UAF Origami super-users listed at the end of this presentation. They will be happy to assist you.

After the Report is Filed • Expect contact from EHSRM to initiate an investigation

After the Report is Filed • Expect contact from EHSRM to initiate an investigation of the reported incident or unsafe condition • Also expect contact from the UA System Office of Risk Services (SORS) Claims Adjuster for any reported incident involving a worker’s compensation, property, or other related claim • You are encouraged to provide any additional written documentation , photos, etc. , related to your incident. This information can be uploaded as part of your incident file immediately AFTER you have saved your report.
