3 North Country Children’s Clinic Founded by Volunteers & First Well Child Clinic Held 1971 Prenatal Education Program Founded; Now Known as Healthy Outcomes 1974 1980 WIC Program Funding Sought and Received; One of First in the Country Pediatric Dental Care Services Commence 1989 Pediatric Primary Care Services Commence 1991 School-Based Dental Program Commences 1993 Pediatric & Family Behavioral Health Services Commence; Start of School-Based Health Program 1994 Transitioned to FQHC, now serving all ages as North Country Family Health Center; Went Live with EHR 2010 2012 2019 Telemedicine Program Begins Today, NCFHC operates

5 What is a Federally Qualified Health Center? • A community-based healthcare provider that receive funds from the Health Resources & Services Administration to provide primary care services in underserved areas. FQHCs must meet a stringent set of requirements, including providing care on a sliding fee scale based on ability to pay and operating under a governing board that includes a majority of patient members.
