He was experiencing rebound tenderness in his lower right abdominal quadrant. Following blood work and an emergency C-T scan, his diagnosis of acute appendicitis is confirmed. He is taken to the Operating Room for an appendectomy. He is received on the surgical ward by the licensed practical nurse. The boy is drowsy, teary and complaining of severe post-op pain. Upon review of his chart and his post-op orders it is noted that he is to receive nothing by mouth, has an intravenous running in his right hand of 2/3 1/3 solution at 80 ml/hr, and has not voided in the past 9 hours. He has a small right quadrant abdominal incision with 4 small steri-strips that is dry and well-approximated. What should the LPN do first? Why?
