+ Focusing on the person rather than the disease n “Behavioral symptoms” of dementia are often a way of communicating unmet needs. n What are the particular needs of each family patient? They might differ given the caregiver’s gender and other demographics. n Example: caregivers for cognitively demented patients might benefit more from emotional support while caregivers for demented AND frail patients might be concerned with knowledge of nursing skills.

What good can they make out of it? n Increased awareness of the shortness of life, which may make them live more intensely in the present. n Identify meaning in the past (memories), present (daily routines, positive aspects of responsibility) and future (to pass on the patient's lifework).

For example, the caregiver can be asked, “Does your husband help with household tasks? ” In this case, the caregiver should be encouraged to involve her husband in simple tasks such as folding the laundry.

+ Tackling Stress n “As a caregiver, what are your strategies for stress relief? ” n If the caregiver cannot readily provide an answer to this question, this is an area of concern. n Coping strategies: n n emotion-focused: worrying and self-accusation problem-focused: confronting issues and seeking information n Caregivers who use problem-focused strategies have less burnout. n “When something goes wrong with your brother's care, for instance, if he loses control of his bladder, how would you react? ” Emotion-focused responses are “I'd cry” or “I'd put him in a home. ” A problem-focused response is, “I'd call the doctor and find out what's happening. ” n A tendency to use emotion-focused responses should alert the healthcare provider to an increased risk of burnout in the caregiver.

+ Support Groups for caregivers n For many caregivers, much of their burden is related to feelings of loneliness or isolation. n Specific groups, such as the Breakaway program, are designed to supplement traditional support groups by providing informal recreational and social activities with a peer group of caregivers who are experiencing similar stresses. n Adult day services are an excellent source of respite for the caregiver and provide engaging activities for persons with dementia. The local Area Agency on Aging can provide the caregiver with a list of nearby facilities.

What language do we as healthcare providers use to describe the situation of a dementia patient caregiver? n Who is the caregiver of the caregiver? “ Put the oxygen mask on yourself first” n Empowering the caregiver – Acknowledging their ability to carry on so many responsibilities (“sandwiched” younger generation of caregivers) n Addressing the limits of caregivers – Explain signs of burn-out to them so they know when to ask for help and why realistically they can’t do everything on their own n Keeping in mind the resistance of the family system to reorganize itself and change
