What is shared care? �Getting involved in your dialysis care �Having the choice about what you want to do and don’t want to do �Learning about your kidney disease and how it affects you �Learning about your dialysis �Improving your quality of life
How will it benefit you? �It will make you feel more in control and less dependant on others �It may reduce your waiting time in dialysis unit �It will help you to understand what you can do to improve your health �It will help you to understand what is happening when you are dialysing �It will make you feel more confident
What shared care is not! �It is not compulsory �It is not about cutting staff �It is not about pushing for home haemodialysis unless that is what you are aiming for �It is not about leaving you on your own. When you are in need, the staff will be available for you.
It is shared - not self - care
Evidence has shown that patients who are more involved in their chronic conditions have better health outcomes and enjoy a better quality of life.
Some of the benefits include fewer infections and better symptom control. Greater understanding of fluid and dietary restrictions and making healthier choices.
