Needs ◦ Delayed vocabulary ◦ Delayed speech ◦ No other family supports ◦ Little prenatal care, mother used drugs for whole pregnancy, ◦ Delays in fine motor and cognitive abilities ◦ Either a physical delay or abandonment issues (won’t move far from Grandma) ◦ Lack of socialization ◦ Grandparents lack nutrition awareness and parenting skills

Goals Speech/Language Goals ◦ By February 2015, Jimmy will be able to say his name when prompted 3 out of 4 times. ◦ By April 2015, Jimmy will be able to clearly verbalize 75 words throughout program day. Cognitive Goals ◦ By. February 2015, Jimmy will be able to do activities on his own with realizing Grandma or a trusted adult is nearby but not interacting with him. ◦ By March 2015, Jimmy will be able to repeat back 5 numbers when prompted 3 out of 4 times.

Goals ◦ Fine-Motor Goals ◦ By December 2014, Jimmy will be able to make marks with markers/pencils/or crayons when given materials 70% of the time. ◦ By May 2015, Jimmy will be able to use a pincer grasp to hold smaller objects such as markers and crayons when handling small objects 50% of the time. ◦ Social-Emotional ◦ By March 2015, Jimmy will be able to seek out an adult and ask for assistance when needed in 3 out of 4 consecutive instances. ◦ By April 2015, Jimmy will seek out a peer to interact with during free play time 2 out of the 4 days of the week.

Daily Schedule 8: 30 -8: 55 Drop off Students take off their coats and put their backpacks away. After all of their materials are away they will sit on the carpet. *Throughout the day the teacher will be listening for Jimmy to clearly verbalize 75 words as well as complete activities on his own while his Grandma and a trusted adult are nearby. 9: 00 -9: 20 Circle time Students will be sitting on the carpet. The teacher will lead the students in a welcome song and talk about the weather. 9: 30 -9: 40 Snack 9: 40 -10: 40 Centers The teacher will use this time to observe and collect data on the students. 10: 40 -11: 10 Small Group/Large Group (small group on Monday and Wednesday, large group Tuesday and Thursday) 11: 10 - 11: 30 Get ready to go home/Pick up The students will clean up and get their backpacks and all of their snow stuff and come sit on the carpet. The paras an teachers will pass out folders for the parents, students will put them in their backpacks. Students will wait by the carpet for their parents to pick them up.

Monday 8: 30 -8: 55 Drop off 9: 00 -9: 20 Circle time We will read the book “I See Winter” by Charles Ghigna. 9: 30 -9: 40 Snack 9: 40 -10: 40 Centers ◦ Science: ◦ Create Snow – Allow students to get their hands messy and explore different textures, Students will make snow out of materials provided by the teacher *Jimmy’s Goal: Seek an adult for assistance ◦ Art: ◦ Create Snowmen –Place cotton balls onto pre cut snowman shapes *Jimmy’s Goal: using pincer grasps to hold smaller objects ◦ Blocks: Free play center *Jimmy’s Goal: Complete activity on own with adult nearby

Monday Continued 10: 40 -11: 10 Small Group During small group today we will split the class into two groups. There will be 7 students in each group. One group will focus on art. They will make a snowfall painting by using pencils. The other group will be working on literacy. The teacher/para at this station will have snowflakes with big black letters on them. They will hold up a snowflake and the students are going to guess what letter it is. The students will be at each station for fifteen minutes before switching to the other station. **While Jimmy is at the art station he will be working on his fine motor goal, Jimmy will use his pincer grasp to hold a pencil. **When Jimmy is at the literacy station the teacher/para will be looking to see Jimmy seeks out an adult and asks for assistance when needed (Social Emotional). 11: 10 - 11: 30 Get ready to go home/Pick up We will sing “I’m a little Snowman” to the tune of “I’m a Little Teapot. ” I’m a Little Snowman (Sung to: I’m a Little Teapot”) Submitted by Kelly I’m a little snowman short and fat. Here is my scarf and here is my hat. When the snow is falling come and play. Build a snowman every day.

Tuesday 8: 30 -8: 55 Drop off 9: 00 -9: 20 Circle time We will read “Over and Under the Snow” by Kate Messner. 9: 30 -9: 40 Snack 9: 40 -10: 40 Centers ◦ Writing: Exploration of Writing Utensils – ◦ Students will explore different writing utensils and test them all out on paper *Jimmy’s Goal: Make marks with writing utensils ◦ Dramatic Play/Dress Up: Open Center – *Jimmy’s Goal: Seek peer to interact with during free play time ◦ Puzzles: “Do you want to build a snowman? ” busy bag – ◦ Children have busy bag with tools to build a snowman *Jimmy’s Goal: Do activities on own with realizing trusted adult is nearby but not interacting with him

Tuesday Continued 10: 40 - 11: 10 Large Group ◦ The children will be asked to draw their favorite thing to do in the snow. They will be able to use colored paper, crayons, markers, and scissors. Each student will make one page for a book to be put together by the class. The students will be encouraged to share their favorite things with their friends. ◦ *This will work on Jimmy’s goals of using writing utensils and seeking peers to play and talk with. 11: 10 - 11: 30 Get ready to go home/Pick up ◦ There Was a Little Snowperson song/finger play ◦ There was a little snow person and there she sat (point at the floor). There are her buttons and there is her hat. When the sun came out. She couldn't stay. Slowly, she just melted away.

Wednesday 8: 30 -8: 55 Drop off 9: 00 -9: 20 Circle time The teacher will read the “Five Little Gingerbread Men” poem. She will have a plate and five plastic gingerbread men. As she reads the poem she will take a gingerbread man away to follow the poem. Five Little Gingerbread Men Five little gingerbread men lying on a tray, One jumped up and ran away. Shouting, "Catch me, catch me if you can. . . I'm really quick, I'm a gingerbread man!“ count down the numbers repeating the stanza above— No more gingerbread men lying on a tray, They all jumped up and ran away. Oh, how I wish they had stayed with me to play. Next time I'll eat them before they run away. 9: 30 -9: 40 Snack

Thursday 8: 30 -8: 55 Drop off 9: 00 -9: 20 Circle time The teacher will read “The Mitten” By David Theoharides. The teacher will have a mitten. She will also have plastic animal toys that will represent all the animals that fit in the mitten. The 8 animals will be given to eight students. As the animal is mentioned the students will put them in the mitten. (Animals include: a mule, a rabbit, a hedgehog, an owl, a badger, a fox, a bear and a mouse) 9: 30 -9: 40 Snack 9: 40 -10: 40 Centers ◦ Writing: Do A Dot Letters – ◦ Stamp out the letters of the students name using Bingo stampers. The letters will be traced out using snowflake shapes *Jimmy’s Goal: Use pincer grasp to hold smaller objects ◦ Dramatic Play/Dress Up: “Frozen Puppet show” – ◦ Create characters from “Frozen” using paper plates and then have puppet show *Jimmy’s Goals: Seek out peer to interact ◦ Puzzles: Printable Mix and Match Holiday Puzzles – ◦ Students will work on own to create fun and silly holiday characters *Jimmy’s Goal: Do activities on own without interacting with adult

Thursday Continued 10: 40 -11: 10 Large Group ◦ Today we will be working on counting. We will be measuring ourselves with paper mittens. We will see how many mittens long we are. When we are done we will record the number to finish the sentence for our class book. Students will work together to measure their friends. We will sit in our circle and one student will lay on the floor while two friends put mittens by them. The rest of the class will be counting as each mitten is being placed by their friend. *This activity aligns with Jimmy’s Cognitive goal, Jimmy will be able to repeat back 5 numbers. As well as both of his fine motor goals, Jimmy will be able to make marks with a marker/pencil/ or crayon and use his pincer grasp to hold small objects. Jimmy will also be working in with his peers to complete this activity. 11: 10 - 11: 30 Get ready to go home/Pick up ◦ The students will clean up and get their backpacks and all of their snow stuff and come sit on the carpet. The class will sing the song “Cloudy Day” Cloudy Day Submitted by Amy The day is cloudy and the wind is bold. Dress up warmly, you mustn't get cold! Put on your coat and zip it up tight, put on your left boot, put on your right. Put on your scarf and put on your hat, put on your mittens and clap! Go outside and play. Come in again, and then we'll say Take off your coat that was zipped up tight, take off your left boot, take off your right. Take off your scarf, take off your hat, take off your mittens, and then take a nap! Note: Students will act out which body part each object goes while it is being song.
