Eligibility Criteria To apply for level 1, pharmacies must: • Deliver tier 2 Stop

Eligibility Criteria To apply for level 1, pharmacies must: • Deliver tier 2 Stop Smoking Service at least one trained Stop Smoking Adviser, service available >80% of opening hours, must have set a minimum of 24 quit dates in the previous year • Deliver Medicines Usage Reviews

Eligibility Criteria Level 2 (to follow) To apply for level 2, pharmacies must: • Increase Stop Smoking Service - two trained advisers, quit rate of 45% since award of level 1 • Increase EHC availability to 100% (exemptions will be considered for exceptional circumstances on application) • Increase NHS Health Checks 100% availability and increase baseline activity by 10% since the award of level 1 • Deliver three of the five optional services listed above at level 1
