Introduction: Sister Joanna Bruner Family Medicine Center - 7, 500 patients in our panel - 40% are uninsured - We have a high level of cultural and language diversity - We are a low resource clinic with a “Do more with less” ethos

What Is The Hook? • My journey to improve our clinic has led me to this simple conclusion: • I can not do this on my own!!! • The model of the “heroic leader” does not apply to our process of transforming into a PCMH. • Has anyone else had that experience?
