5 Year ACC Strategy Map, Strategic Themes and Goals MISSION To transform cardiovascular care and improve heart health VISION The members of the College will dramatically reduce the incidence, severity, and complications of cardiovascular disease as we promote prevention, reduce disparities in health care, and improve personal populationbased cardiovascular health. CORE VALUES STRATEGIC THEMES Population Health Purposeful Education ACC is a leader in efforts to reduce the burden of cardiovascular disease in US and global populations. Transform CV workforce by developing a personalized, competency-based educational experience; producing a curriculum that addresses clinical, administrative, and leadership skills; and engaging and expanding the community of learners. Transformation of Care Cardiovascular specialists choose ACC as their professional home. Patients receive the highest quality, patientcentered, cost-effective CV care with improved outcomes. Desired Outcomes from ACC’s Strategic Goals STRATEGIC GOALS DESIRED OUTCOMES Perspective: Members /Stakeholders Improve (population) heart health • Reduce the global burden of cardiovascular disease • Achieve goals of the Triple Aim Improve cardiovascular (CV) competency • CV physicians accurately evaluate, diagnose and manage acute CV conditions and chronic CV diseases • CV care team members (non-physicians) promote CV wellness and contribute to the team's management of acute and chronic CV diseases Increase member engagement and value • CV Specialists choose ACC as their professional home Increase adoption of clinical policy and best practice • Cardiovascular care professionals are practicing within policy and best practice which consider cost, value or resource utilization. • Members use and value ACC resources Perspective: Stewardship Increase financial sustainability • Ensure that the ACC has sufficient resources to execute the mission of the College until CV disease has been eradicated. ACC Sports and Exercise Cardiology Section Supports ACC’s Strategic Plan MISSION To support the College’s mission through section member efforts to improve heart health in athletes and exercising individuals, and promote exercise for primary and secondary prevention of CV disease. How the Sports & Exercise Cardiology Section supports the College’s STRATEGIC THEMES: Population Health Purposeful Education S&EC Section promotes global CV and overall health during sport and exercise activities in all populations – in all ages those healthy or with CV disease. S&EC Section is the leading provider of resources and information on CV care of athletes and exercising individuals, fostering enhanced connectivity, coaching and collaboration within the sports and exercise cardiology community. Membership Value and Engagement CV specialists choose the S&EC Section as their primary community of practice as they pursue optimal CV care of athletes and exercising individuals of all ages. Transformation of Care Athletes and exercising individuals receive the highest quality, patient-centered, costeffective CV care with improved outcomes because of the work of S&EC Section. Desired Outcomes from the efforts of Members and Leaders Sports & Exercise Cardiology Section: Reduce the global burden of cardiovascular disease by encouraging sport and exercise for primary and secondary prevention. CV specialists evaluate, diagnose, and manage return-to-play recommendations based on patient-centered, evidence-based medicine. Sports cardiology professionals choose ACC as the premier source of trusted information in CV care of athletes and exercising individuals. Section works with the College in establishing policies for the care of athletes and exercising individuals, and care providers are practicing within policy and best practices that consider cost, value or resource utilization. • Section works with the College to create consensus statements on (a) optimal exercise dose in primary and secondary prevention and (b) ECG interpretation in athletes and other exercising individuals. • Section offers robust Mentoring and Sports and Exercise Cardiology Learning Exchange programs. • Section serves as a resource for providers interested in starting new local programs in Sports and Exercise Cardiology. • •

Alignment of Sports and Exercise Cardiology Section’s Activities to ACC Strategy STRATEGIC GOALS SPORTS AND EXERCISE CARDIOLOGY SECTION’S INITATIVES AND ACTIVITIES Perspective: Members /Stakeholders Improve population heart health • Reduce the global burden of cardiovascular disease by encouraging sport and exercise for primary and secondary prevention. • Achieve goals of the Triple Aim through evidence-based symptom management. Improve cardiovascular competency in care of athletes and exercising individuals • Care team members responsible for pre-participation exams make responsible decisions. • CV specialists evaluate, diagnose, and manage return-to-play recommendations based on patient-centered, evidence based medicine. Increase member engagement and value • Sports cardiology professionals choose ACC as the premier source of trusted information in CV care of athletes and exercising individuals. Foster creation and adoption of clinical policy and best practice • ACC establishes policies in the care of athletes and exercising individuals, and care providers are practicing within policy and best practices that consider cost, value or resource utilization. • Members and non-members interested in sports and exercise cardiology use and value ACC resources. Perspective: Stewardship Increase financial sustainability • Partner with other organizations, both public and private, who foster exercise as integral part of population health on broad scale. • Provide extraordinary value for section dues. Protect brand reputation • Protect and ensure the reputation of ACC by convening multidisciplinary groups to actively participate in dialogue regarding public policy with state, federal and international decision-makers on role of cardiology experts in care of athletes and exercising individuals. Perspective: Internal Processes Improve education development & delivery for care team including first responders • Develop educational plan including engagement in ACC. XX, live and digital formats, and domestic and international chapters. • Create core curriculum for CV professionals to provide care at basic, intermediate and advanced levels within the care team Enhance member experience and encouraging new and renewed participation • Offer learning opportunities connecting beginners with experts. • Offer activities to support peer networking and personalized coaching. • Provide robust clinical community resources on acc. org, JACC family publications, newsletters, ACC In Touch social media, etc. Improve development & delivery of CV standards • Develop metrics for causes and incidences of SCA/SCD/and other CV outcomes (MI, arrhythmias, etc. ) with registries and QI initiatives. • Better define norms in American “athletes”. • Create consensus statement on optimal exercise dose in primary and secondary prevention. • Create consensus statement on ECG interpretation in athletes and other exercising individuals. • Promote research opportunities especially data driven management algorithms. • Create toolkits in advocacy, media relations, pre-participation screening for CV disease, and digital apps to support eligibility guidelines. Perspective: Organizational Capacity Increase governance and decision making effectiveness • Seek feedback and welcome members’ participation in project development via work groups and task forces building the leadership pipeline. • Encourage member and staff connections across divisions of the College to foster efficient decision making. Increase staff & member knowledge and skills • Provide robust Mentoring program and Sports and Exercise Cardiology Learning Exchange program. • Serve as an incubator for providers interested in starting new local programs in sports and exercise cardiology. Improve section health through innovation • Capitalize on the wearable technology and mobile device diagnostic tool movements to encourage overall physical health. Better utilize shared services and tools • Work within Section Steering Committee to learn and share experiences with other ACC Sections.
