6300. Personnel Administration Guide, 6370 Personnel Action Forms C. How to Use the Personnel Action Form Only an office manager or hotel supervisor (including a department manager and general manager) may prepare a Personnel Action Form. Initiate the form and submit it to the general manager for approval. Do not tell a team member about a wage-rate change or any other change in status that requires the general manager’s approval until an approved Personnel Action Form has been returned to you from the general manager. After approval, the general manager sends the PAF original to the administrative office. If the PAF contains a change about which the supervisor is to inform the team member, the general manager gives a copy of the approved PAF to the supervisor. Recordkeeping and Distribution The general manager (or, where applicable, the office manager) files Personnel Action Forms in the administrative office in each team member’s employment file. Treat all information contained on PAFs confidentially. Create a History record in HR Central for all events recorded on PAFs. Store PAFs electronically on MMI File. Spot®.
