About Logisti. Care Solutions LLC April of 2012 Logisti. Care began managing the Non Emergency Medical Transportation contract for Texas Health and Human Services SDA 1 Region. The SDA 1 Region includes fourteen counties: Dallas, Denton, Ellis, Erath, Hood, Hunt, Johnson, Kaufman, Navarro, Palo Pinto, Parker, Rockwall, Somerville, and Tarrant.

Contract Responsibilities Non Emergency Medical Transportation is for Medicaid eligible clients who does not have transportation to and from their medical appointments. Logisti. Care is contracted with over sixty transportation providers, providing daily oversight of all drivers and vehicles for safety and compliance.

Contract Responsibilities Transports over 18, 000 unduplicated riders a month Transporting members by rural transit operations, commercial providers, cabs, fixed route, and paratransit bus systems Mileage reimbursement can be provided for members who transport themselves, have a family member, or friend to transport them to and from medical appointments

Eligible Members, Covered Services, & Responsibilities Medicaid recipients, Children with Special Health Care Needs,

Eligible Members, Covered Services, & Responsibilities Medicaid recipients, Children with Special Health Care Needs, and Transportation for Indigent Cancer Patients members who live in any of the SDA 1 counties Have no other resource for transportation Going to a Medicaid-covered service that will be provided by a Medicaid-enrolled provider Book trip or trips at least 48 hours in advance (2 business days) or up to thirty days in advance

Levels of Covered Services Mass Transit The member lives within ¼ mile of a bus stop and the medical facility is within ¼ mile of a bus stop. The member can walk ¼ mile and understands common signs and directions. Ambulatory: The member can walk unassisted from the doorway to the curb. The member can walk unassisted from the vehicle to the facility.

Levels of Covered Services Wheelchair The member is confined to a wheelchair The member cannot transfer from the wheelchair to the seat of an ambulatory vehicle Gas Mileage Reimbursement for the member, family member, friend, neighbor, etc. who is transporting the member in a personal vehicle. They can be set up as an Individual Transportation Provider (ITP) Meals and lodging can be obtained for members who meet certain criteria.
