General Reminders • Please do NOT open Web. DCUTM in multiple tabs or windows! • Please remember to enter screen failures • CRF data must be submitted within 5 days of collection • Exceptions: • Adverse Events must be submitted within 5 days of knowledge of the event • Death must be reported within 24 hours of knowledge of the event • DCRs should be responded to within 5 days • Please remember to respond to the DCR after updates have been made, and be sure to submit the CRF so that we can close the query. • If you do not have access to the data, then please wait until you do have access to respond and update the query.

DOAs • Please remember to keep your DOA up-to-date • This prevents us from contacting the wrong individuals • All email recipients are pulled directly from the currently accepted DOA. • A DOA is only sent for review once [DOA complete = No] • Only someone currently on your DOA with [RDC] or [N – Maintain essential regulatory documents] can update the DOA

Baseline F 143 • Please remember that the NIH score on F 143 at the baseline visit should be the same one used to randomize a subject on F 102. • The NIH assessment used should be before rt-PA administration • If we have queried you to update F 143 because it does not match the on F 102, remember to update the entire form and not just the score at the end. • If the assessment at baseline was done by a non-study team member, then please enter in Qd 1 and Qd 2 the individual on the DOA with [F: Administer NIH Stroke Scale] who verified the NIH score should be listed.
