Lockout A technique used to prevent hazardous energy from escaping. A padlock is placed on a disconnect switch, circuit breaker, valve handle or other energy-isolating mechanism that is in the off or closed position.

Tag Out Energy isolating device is placed in the safe position and a written warning tag is attached.

Six Steps ¨ 1. Prepare for shutdown ¨ 2. Shut down ¨ 3. Isolate equipment ¨ 4. Apply lockout / tagout device ¨ 5. Control STORED energy ¨ 6. Verify Isolation of equipment ¨ Know energy types and ¨ ¨ ¨ hazards Do it by the book Cut off EVERY energy source feeding it. At all disconnects, valves and energy isolating devices Relieve disconnect or restrain residual energy that COULD be present Don’t take anything for granted. Double check !

Removing locks and tags 1. Make sure equipment is intact 2. Remove tools and non-essentials 3. Tell affected employees (head count) 4. Make sure area is clear 5. Remove lockout /tagout devices (Each worker removes own lock/tag) 6. Sign and turn in tags 7. Tell affected employees tags are off and equipment is ready for operation

Removing Locks / Tags of an Authorized Employee NOT Present ¨ Foreman of the work crew can remove the lock / tag of an authorized employee only under the following conditions: 1. 2. 3. 4. A thorough visual inspection conducted and ensure the work area is clear and authorized employee is not present The foreman shall verify the authorized employee is not on campus (NO LOTO shall be removed unless its confirmed the employees not present All reasonable efforts have been made to contact the authorized employee The foreman shall ensure the authorized employee is aware of the device being removed
