Dementia (post diagnostic support) April 2016 Update This Months Progress Objectives for the dementia post diagnostic programme 1. Scope the current access to and interventions by 5 of the allied health professions after diagnosis. 2. Implement a change programme in 2 NHS boards/local authority areas, with the objective to enhance self management and access to AHP services 3. Complete literature review of AHP contribution to post diagnostic support 4. Scope current training in post diagnostic support 5. Embed the role of allied health professionals and post diagnostic support to evidence based policy document commitment 4 (Scotlands Dementia strategy) 1. First PDSA cycle commenced in Dumfries & Galloway around testing of DLW Framework and Measurement Tool 2. Ongoing work around identification of, and connecting with new project team members 3. Continuing to develop Framework for Dementia Link Workers Accessing AHP Services. Linking with Project Team members re detail 4. Collation of AHP Self Management resources, Commenced development of online platform (closed page on current Co. P). Populating with additional Service information and SM resources. 5. Final webex with Action 2. 6 Board Leads for: Sharing of the findings from the national scoping work with the key messages and recommendations Provide an update on the progress of the Improvement work currently taking place in Dumfries & Galloway and Edinburgh Outline the synergies with the new Active & Independent Living Programme, Dementia 3 and changes in primary care This Group now formally disbanded 6.
