Core Competency 10 � Engage, assess, intervene, and evaluate with individuals, families, groups, organizations, and communities. -Look at the residents charts and ask members of the staff for additional information.

Practice Behavior 10. 2 � Use empathy and other interpersonal skills. - I make sure that I’m listening to the residents wants and needs.

Practice Behavior 10. 3 Ø Develop a mutually agreed-on focus of work -My supervisor and I always have a set plan for everyday on what is going to be happening. I make sure I get my tasks done and ask is there anything more I can do to help

Practice Behavior 10. 4 � Collect, organize, and interpret client data. -Attend care plan meetings with my supervisor and assist on collecting information, organizing, and talking about effective treatment for the residents.

Practice Behavior 10. 5 � Assess client strengths and limitations -Look at clients strengths and weaknesses at care plan meetings.

Practice Behavior 10. 6 � Develop mutually agreed-on intervention goals and objectives -Set achievable goals for the residents to reach.

Practice Behavior 10. 7 � Select appropriate intervention strategies -Get resident involved with their own care plans and their families as well.

Practice Behavior 10. 8 � Initiate actions to achieve organizational goals -Work on discharge paperwork with my supervisor for residents leaving the facility.

Practice Behavior 10. 9 � Implement prevention interventions that enhance client capacities -Educate the clients the importance of sticking with their home health agencies routines even after they are discharge.

Practice Behavior 10. 10 � Help clients resolve problems. -Find the best possible solutions for the residents.

Practice Behavior 10. 11 � Negotiate, mediate, and advocate for clients. -Residents can file grievance reports and they will immediately get investigated.

Practice Behavior 10. 12 � Facilitate transitions and endings -Let the residents know the status of their cases.

Practice Behavior 10. 13 � Social workers critically analyze, monitor, and evaluate interventions -Making sure that the treatment I proposed is following through with the resident.
