Consumer’s Care Plan Each consumer at each residence has a written care plan. This

Consumer’s Care Plan Each consumer at each residence has a written care plan. This training session will discuss the different sections to the care plan. It will also discuss how to implement the care plan to provide care to that consumer. Each care plan is created so the employee has a general idea what the needs of the consumer are before they enter the residence. They are also created to inform the employee what care “Family Care” expects to be completed while at the residence with the consumer.

Match Criteria This section of the care plan is the consumer’s “Match Criteria”. This

Match Criteria This section of the care plan is the consumer’s “Match Criteria”. This will provide to you all of the consumer’s requirements needed to complete the care that the consumer requires.

Care Needs / Demographics / Activities of Daily Living (ADLs) Care Needs: This section

Care Needs / Demographics / Activities of Daily Living (ADLs) Care Needs: This section will indicate an overview of the duties that must be done while at the residence. Demographics: This section will give you all the information related to the consumer’s age, birthday, marital status, language, etc. . Activity of Daily Living: This section will provide you the consumers daily self care activities that you may need to assist with such as meal preparation, ambulation & transfers, toileting, etc….

Instrumental Activities of Daily Living (IADLs) / Medical Condition Instrumental Activities of Daily Living

Instrumental Activities of Daily Living (IADLs) / Medical Condition Instrumental Activities of Daily Living (IADLs): This section explains the activities that you may need to assist the consumer with once they are up, dressed, and put together. This may include cleaning the living areas, laundry, dishes, etc… Medical Condition: This section gives you insight of medical conditions the consumer may have, to help you understand what assistance the consumer may need.

Mental/Behavior Conditions / Elimination Mental/Behavior Conditions: This section will inform you if the consumer

Mental/Behavior Conditions / Elimination Mental/Behavior Conditions: This section will inform you if the consumer is alert and oriented or if they have any conditions that may effect the care needed for the consumer. Elimination: This section will inform the aide if the consumer is continent or needs additional help or supplies to ensure the consumer is clean and dry throughout the day.

Medications & Supplements / Ambulation / Bathing, Grooming, & Dressing Medications & Supplements: This

Medications & Supplements / Ambulation / Bathing, Grooming, & Dressing Medications & Supplements: This section will inform the aide the times to remind the consumer to take their medication. Remember, you are only allowed to “remind” the consumer to take their medications. Ambulation: This shows the aide if the consumer is able to walk independently or needs assistance or any assistive devices. Bathing, Dressing, & Grooming: This will inform the aide the method & frequency of the consumer’s hygiene habits. It will also, inform the aide if any special instructions are needed.

Equipment / Environment: This section will inform the aide if any assistive equipment is
