The stressors usually fall into one of 8 categories. The LEAP will help you identify the challenges that most need your attention. It will help us in our work together to prioritize, plan and take action. What to Do Explain to the client how to take the LEAP Assessment and how to score it. Then ask the client to return it to you when done so you can review the results together.

The client is very concerned. If the total is 16 -24. The client is concerned. If the total is 8 -15, the client is not overly concerned. Instruct the client to look at the reverse side to pursue actions to mitigate the areas of concerns.
