Year 12 (HSC) Assessment �All formal assessment task are used to determine an assessment

Year 12 (HSC) Assessment �All formal assessment task are used to determine an assessment mark. �Students must sign to say they received an assessment task as well as sign to say they have submitted an assessment task. �School assessment marks are sent in to the NESA. �These assessment marks create data for NESA and determine rankings for each course.

The Assessment Schedule �All students receive an assessment schedule in their student handbook. �The assessment schedule outlines their formal assessment tasks for the year. �It includes outcomes for the task, the week the task is due to be submitted and the weighting for each task. Note: Students should use this information provided in the assessment schedule to help organise themselves.

Your responsibility as a student �Assessment tasks must be completed by the due date �All student work must be their own work �All work (including assessment tasks, class work and homework) must be completed �A genuine attempt must be made for each task �School attendance is an essential requirement �Poor attendance may indicate a lack of due diligence and sustained effort on the student’s part

What does a NESA Warning Letter and an ‘N Determination’ mean? � The NESA

What does a NESA Warning Letter and an ‘N Determination’ mean? � The NESA warning letters specifies exactly the tasks the student needs to complete to satisfy course requirements. These tasks must be attended to promptly � If a student has received at least 2 or more warning letters, and work remains outstanding or the student has received a zero for 50% or more of their assessment they may be given an ‘N Determination’ in that subject � An ‘N Determination’ means a non-award for a subject. If the loss of the course takes the student below the number of units required, i. e. 10 units, then the student will not receive their Preliminary HSC. � If you are N-Determined in English then you DO NOT receive a Higher School Certificate

What if I am sick or cannot submit the task on the due date? �Submit FORM A ‘Illness Form’ form signed by a parent / caregiver (p 35) �Submit official medical documentation with the form e. g. Doctor’s Certificate, Hospital documentation �If appropriate forms are not signed and there is no satisfactory evidence stating illness, the student will receive zero for that task.

What happens if there are extenuating circumstances that mean I cannot submit the task or impacts on my performance? �Submit Form B ‘Misadventure Absence Form’ signed by a parent / caregiver (p 36). �Submit official documentation depending on the reason for the misadventure /absence e. g. Police Event Number, evidence of a car accident on the way to school. �If you know you will be absent due to TAFE or other circumstances, ensure you complete the documentation before the task and submit it to your teacher.

What if I am not happy with a teacher’s decision about an assessment task? �Complete FORM C, the ‘Appeal Form’ in the student handbook (p 38). �Clearly state your case. �Ensure that a parent signs the form. �The appeal will be heard by the Principal (or nominee) and two other executive members of staff. �A decision will be made promptly.
