The YCOEM Form • The person ‘taking’ the message puts their name in the “Taken By” area. If taken by amateur radio the operator should also put his/her callsign. • Name and Position of person responding to the message goes at the bottom of the “Reply/Action” area along with the time and date of the reply. When taking or sending message traffic always work from the upper left to lower right on the form that way messages can be copied/sent without a form using plain paper. .
