It should be considered as second-line therapy in men in whom PDE 5 inhibitors are ineffective, are contraindicated, are not used properly or have caused adverse reactions. We believe intraurethral alprostadil to be a valid noninvasive therapy in patients with ED who have failed therapy with PDE 5 inhibitors, or in those for whom PDE 5 inhibitors are contraindicated.
