Enquire about falls in the previous year and prior fragility fracture Is the stage 1 Falls & Fracture screen positive? Yes Patient added to Falls Register Health & Well-Being Advice Register Stratification Number of Patients under the care of Community Provider Local Healthcare Teams Number of patients living in a 24 hr Care setting Number of housebound patients (Not currently Under the care of LHc.
