Incidents Findings Below are some findings from 2020 incidents. LTI#8 – 7 th May 2020, 1: 30 Hrs. What happened? Activity was to remove the pin between the mast cylinder and support cylinder. Assistant Driller (AD) was using Mobile Elevated Working Platform (MEWP) for accessing the cylinder. Assistant Driller came out of basket while attempting to hammer the pin, he slipped striking his rib cage on beam. After initial First Aid by the rig medic, Assistant Driller was taken to Ibri hospital by the rig ambulance. X-ray revealed fracture of ribs How to prevent this at your site? Ø Ø Ø Do you always take time to correctly position MEWP for full use of basket? Do you always adhere to working at height / MEWP Procedure requirements Do not take the short cut. Any changes in job plan, stop and reassess. Intervention must be practiced by all crew members at all times. Do you always use lanyard and safety harness correctly with dedicated anchor point? Do not climb out of MEWP basket while working at height
LTI – 17 th May 2020, 11: 00 Hrs. What happened? Hose Deployment Unit (HDU) crew were performing drill water hose retrieval activity. While the HIAB truck operator was hoisting and moving the hose reel lifter attached to the loader crane, it got stuck in the handrails of the HIAB truck. The Assistant Technician who was standing on the trailer parked adjacent to the HIAB truck was trying to free the stuck he got his right hand index finger crushed between the hose reel lifter plate and the handrails of the HIAB truck. How to prevent this at your site? Ø Ø Always ensure Supervisors, supervise the job? Always, Identify and Communicate all hazards, their Risks & consequences prior to start work. Remind Stop Work Authority Always INTERVENE, stop and correct unsafe acts and conditions Always ensure Hands –off methods are used to maneuver and manage hoisted loads or accessories Always keep Hands & Fingers away from line of fire
LTI#16 – 21 st August 2020, 10: 30 Hrs. What happened? A third party Carpenter was working in the kitchen to install aluminum frame in the kitchen. He used angle grinder to cut an aluminum angle bar to fit on the frame. While doing so he used one hand to hold the material and second hand holding the grinder. Half way cutting through the aluminum angle bar, grinder lost contact with the aluminum piece and it came in contact with his genital area. First aid was administered by site medic and then transferred him to Haima hospital from where he was transferred to Nizwa hospital for further diagnosis and treatment. How to prevent this at your site? Ø Ø Ø Always ensure only qualified contractor personnel work on site, How you ensure this is happening? Do you always ensure PTW is in place prior doing the job? Do you always ensure appropriate tools are available for the task? Do you always ensure your body is out from line of fire while using portable cutting equipment? Do you always ensure supervisor is supervising the job? Ensure designated work place is used for cutting / grinding activity
Fatality (LTI#23) – 17 th Sept 2020, 18: 00 Hrs. What happened? On 17 th September 2020 at approximately 18: 00 hours a fatal incident occurred during de-mobalization of TOCO assets from Rig 72 move on the Fahud jebel back to Fahud Pac. While driving a low bed trailer back from rig move operation and carrying a crane up hill (Jabel), the driver lost control of the vehicle when it started to roll backward due to unknown reason. The driver jumped out of the vehicle while trying to escape and sustained fatal head injury. He passed away on the way to PDO Fahud clinic. Tier 2 #3 (Off Plot) – 14 March 2020 PROCESS SAFETY EVENT: What happened? 12” Pipeline Blind Flange Leak At a Remote Manifold Station a civil sub-contractor detected a leak by hearing a sound from inside the station. An operator visited the site and confirmed a leak from a blind flange in a dead leg of a 12” loop line. All connected wells were isolated and the line depressurized. The flange was opened, and both gasket and blind flange, showing signs of erosion-corrosion damage from scaling, were replaced. It appeared that earlier signs of a potential leak (through salt deposition found around the flange) had been observed but were not reported as the threshold limits of weep/seep leakages had not been met. How to prevent this at your site? Ø Ø Do you routinely check remote manifolds and other off plot assets for early leak symptoms. How do you ensure this is happening? Always report any indication of a leak even if it does not meet the weep/seep criteria Report any leak, even if it does not meet the weep/seep criteria Leaking point
Tier 2 #5 – 25 May 2020 PROCESS SAFETY EVENT: What happened? Crude Tanker Overfill During loading of crude oil from storage tanks into crude tankers, as the filling of the 1 st tanker had been finished, the operator started filling a 2 nd oil tanker from the 2 nd loading point, but forgot to close the isolation valve to the first (full) tanker. At a certain moment the tank supplying the crude to the 2 nd tanker ran empty and another tank had to be lined up. This also required the use of another loading pump. The discharge of this pump was lined up via a crossover valve to reach the 2 nd truck. However, that loading pump was also lined up to the 1 st tanker that was already full but had the valve left open, which resulted in an overflow of the tanker. How to prevent this at your site? Ø Ø Always before lining up equipment ‘walk the line’ to verify and validate the line up change. During crude tanker truck filling the level shall be regularly monitored Do not leave a critical transfer unattended (PSF-2)
Tier 1 AIPS#01 • Occurred on 17 th August 2020 at the Mabrouk Remote Manifold Station (North Manifold-2) in the Qarn Alam area. A release of hydrocarbon gas occurred from a long hairline crack on the pipe side from the toe of a weldolet joint connecting a 2” vent valve to a 6” piping spool upstream of Relief Valve. Tier 1 AIPS#02 • Occurred on 31 st August 2020 at the Saih Rawl Oil Station in the Qarn Alam area. A leak occurred on an old clamp installed at the inlet manifold which resulted in a release of hydrocarbon oil and water. Tier 1 AIPS#03 • Occurred on 21 st September 2020 in the Lekhwair area. A release of hydrocarbon occurred at the Wellhead of the Lekhwair-979 well.
