Baseline Current re-hospitalization rate is 4% within 30 days of discharge AIM Big Aim: Reduce to 3% or less Little Aim: Enhance more individuals to be involved in support calls.
Rapid Cycle Changes April 2012 • Action: Clients shown video, asked to sign up • Study: Tape too long, 4 clients signed up for program May 2012 • Action: New shorter video • Study: 6 clients signed up for program, 0% were rehospitalized June 2012 • Action: Change how we are asking about support calls • Study: 22 clients signed up for support calls, 10% were rehospitalized July 2012 Action: Continuing to change how we ask about support calls Study: 30 clients sign up, 7% rehospitsalized August 2012/September 2012 • Action: Continue to change asking about support calls • Study: Individuals who sign up and participated in support calls, 2% rehospitalized, Clients who signed up and DID NOT follow through 9% rehopsitalization.
Impact 1. The goal was 3% or less re-hospitalizations. 2. 3. 4. 5. Clients that choose to participate averaged 2% re-hospitalization. Clients who choose not to participate averaged 9% readmission rate. Clients feel support upon discharge. Enhance staff morale. Better understandings of other departments. Increased emphasis on hand offs and crisis planning.
