Alzheimer’s Disease Program Initiative (ADPI) • Competitive grant from USDHS, Administration for Community Living • $636, 000 in federal funds over three years: 2018 -21 (total project $849, 000) • Supports development of a dementia-capable system of care in Rhode Island • Aligned with efforts under State Plan on Alzheimer’s Disease and Related Disorders (ADRD) + responsive to Brown University’s ADRD workforce survey 5

What’s Happening? Supporting families + building system capacity, infrastructure Individuals + Caregivers • Expanding Alzheimer’s Association’s Persons Living with Dementia (PLw. D) training to Spanish-speaking populations + offer quarterly PLw. D training to ~120 individuals • Training bilingual Powerful Tools for Caregivers (PTC) workshop leaders + offering PTC with group respite to ~120 care partners • Connecting engaged families to Family Caregiver Alliance and the POINT for additional support • Following up regularly with Carebreaks families to support changing needs + facilitate referral connections 7

What’s Happening? Supporting families + building system capacity, infrastructure (cont. ) Workforce + Systems • Deliver dementia-specific training curriculum to over 200 direct service professionals • Deploy advanced training to providers in receipt of direct service dementia-capable training certificate to more widely spread educational information and tools • Collaborate with RI Geriatric Education Center on recruitment of primary-care practices (PCP) to complete educational training + implement with patient population. Training will be targeted towards physicians and nurse practitioners. • Promote early referral + detection Alzheimer's and related dementias among PCPs • Integrate practices with care planning + encourage care transition to other network services • Collect + analyze data across 9 outcome measures from 16 activity outputs Ex. Medicare claims tracking for CPT code use, Perceived Change Index for PTC recipients, Quality of Life in Alzheimer’s Disease, Sense of Competence in Dementia Care Staff Scale, Qualitative Interview 8

Outcomes? ü Heightened awareness of importance of early detection + documentation of ADRD diagnosis

Outcomes? ü Heightened awareness of importance of early detection + documentation of ADRD diagnosis among healthcare providers, family caregivers, and people living with mild symptoms ü Increased knowledge of best practices for ADRD diagnosis, care transitions planning, and community support referrals through training of the clinical professional workforce ü Stronger capacity to provide person-centered dementia care, including supports for care transitions for people with ADRD through training of the direct service workforce ü Improved quality of life for people living with dementia + their families, caregivers ü Solid foundation for a dementia-capable HCBS system, offering quality care + supports to ADRD population, families + caregivers 9
