Objectives § Classify adults as thriving or in crisis based on aging domains common across disciplines/professions. § Explain the referral process to IHSS by health care professionals and list what qualifies a patient/client for their services. § Explain the responsibility of the health care professional when a patient/client is referred for IHSS. § Describe what can be expected when a patient/client is referred to IHSS. § List the common causes of falls and cognitive impairment, two highly prevalent problems in the older adult population. § Name several early signs of cognitive impairment in older adults identifiable by health care professionals from across disciplines.
Objectives (cont. ) § Name and describe some common assessments and reporting techniques that can be used across disciplines in the assessment of falls risk and cognitive impairment in an older adult. § Explain what occurs in a home safety evaluation and how this information can be used by members of an interprofessional health care team to improve safety for older adults. § Explain what decision making capacity means in the health care setting and how it is assessed by health care professionals. § Explain how a team approach helps to provide solutions to complex geriatric patient needs. § Describe several strategies that can foster productive collaboration among members of an interprofessional health care team.
