PLEASE PLACE ANY NOTES OR MONEY IN THE EMPTY SHEET PROTECTOR. IT IS YOUR CHILD’S RESPONSIBILITY TO RETURN THEIR FOLDER DAILY. FOLDERS ARE CHECKED ON A DAILY BASIS.

RE-TEACHING/ REASSESSMENT A TEACHER SHALL PROVIDE CORRECTIVE INSTRUCTION AND A REASONABLE OPPORTUNITY TO REASSESS FAILURE TO MASTER TEKS ON MAJOR GRADES. THE HIGHEST POSSIBLE GRADE THAT CAN BE EARNED AND RECORDED ON THE REASSESSMENT IS A 70. THE TEACHER WILL MAKE A NOTE IN THE ELECTRONIC GRADE BOOK OF THE DATES AND GRADES OF THE REASSESSMENT. DISCIPLINE COLOR CODE SYSTEM: EACH DAY EVERY STUDENT BEGINS ON GREEN. IF THE STUDENT IS DOING MORE THAN WHAT IS EXPECTED OF THEM, THEY WILL HAVE THE OPPORTUNITY TO MOVE UP TO GOLD. STUDENTS WHO ARE MISBEHAVING WILL FIRST GET A VERBAL WARNING OR HUSKY BUCKS WILL BE TAKEN. IF THE STUDENT'S BEHAVIOR CONTINUES, THEY WILL BE ASKED TO PLACE THEIR CONDUCT CLIP DOWN A COLOR AS A VISUAL REMINDER OF THE UNDESIRED BEHAVIOR. IF THE BEHAVIOR IS CORRECTED THEY WILL BE ABLE TO MOVE BACK TO GREEN. IF THE STUDENT CONTINUES TO MISBEHAVE THEY WILL BE ASKED TO PLACE THEIR CONDUCT FOLDER ON THE TEACHER'S DESK AND WILL RECEIVE A NOTE IN THEIR FOLDER AND A COLOR CHANGE TO BLUE. IF THE STUDENT’S BEHAVIOR BECOME SEVERELY DISRUPTIVE TO LEARNING THEY CHANGE THEIR COLOR FROM BLUE TO YELLOW AND A NOTE, PHONE CALL TO PARENTS, OR A PARENT CONFERENCE MAY BE REQUESTED. IF THE STUDENT HAS INAPPROPRIATE BEHAVIOR AFTER SEVERAL WARNINGS AND COLOR CHANGES, THEY CHANGE THEIR COLOR FROM YELLOW TO RED AND A PARENT CONFERENCE WILL BE REQUIRED AS WELL AS AN OFFICE REFERRAL BASED ON THE LEVEL OF THE OFFENSE. BEHAVIOR AND REPORT CARDS STARTING THE 1 ST NINE WEEKS THE STUDENTS WILL RECEIVE NUMERICAL GRADES: • 90 -100% EXCELLENT PROGRESS “A” • 80 -89% GOOD PROGRESS “B” • 75 -79% WORK IS OF AVERAGE QUALITY “C” • 70 -74% WORK IS OF POOR QUALITY “D” • BELOW 70% FAILURE “F” EACH SUBJECT, (MATH, READING, LANGUAGE ARTS, SCIENCE, AND SOCIAL STUDIES), WILL HAVE A MINIMUM OF 7 DAILY GRADES AND/OR QUIZZES AND 3 MAJOR GRADES EACH NINE WEEKS. EVERY SUBJECT WILL HAVE A MINIMUM OF 10 GRADES ON EACH REPORT CARD. THE REPORT CARD GRADE IN EACH SUBJECT WILL REFLECT A WEIGHT OF 70% DAILY GRADES AND QUIZZES AND 30% MAJOR GRADES.
SOCIAL CONTRACTS During the first week of school, the students will create a class social contract that states how they have agreed to act in the class as well as in the school. .
HUSKY BUCKS Students are able to earn husky bucks for good behavior in the classroom and throughout the school. Students can also loose husky bucks for not following directions, but can earn those bucks back. At the end of every week, the students can turn in their husky bucks to their teacher in order to earn different rewards. HUSKY HANGOUT At the end of every nine weeks, the students can earn husky hangout for having the Letter “S” on all sections of conduct on their report card. Husky hangout is a nice time for all of the students to celebrate their good behavior throughout the nine weeks with the grade level. They will play games, watch movies, or have a technology game day.
REPORT CARD CONFERENCES PARENT-TEACHER CONFERENCES WILL BE HELD ON NOVEMBER 8 TH, 9 TH & 10 TH. ALL PARENTS MUST ATTEND THE CONFERENCE IN ORDER TO RECEIVE YOUR CHILD’S REPORT CARD. VISITORS TO HUBENAK • WHEN VISITING HUBENAK, PLEASE BE SURE THAT YOU ONLY ENTER THROUGH THE FRONT DOOR. • ALSO, YOU WILL NEED TO CHECK IN WITH THE FRONT DESK TO GET A NAME BADGE AND SIGN OUT.
Cafeteria Students may charge meals up to the value of 5 breakfasts and 5 lunches or $15. 00. A low balance reminder will be sent home to elementary students and an automated phone call will occur, for all students, when a negative balance appears on the students account. If a student reaches a charge limit of $15. 00, an alternate breakfast of a cereal and milk will be provided. During lunch service, a cheese sandwich and milk will be provided until the negative balance is paid or 5 alternate meals have been served. After 5 alternate meals, families should make other arrangements for meals during the school day. A la carte items are not available for charging. This policy will be consistent for all grade levels on all campuses. • Parents can put funds on the student account prior to meal service. A SNACK TIME WILL BE A SET TIME EVERY DAY AFTERNOON AT 12: 05 P. M. PLEASE MAKE SURE YOUR CHILD IS BRINGING A HEALTHY SNACK. PLEASE NO UTENSILS. THE TEACHER WILL NOT PROVIDE SNACKS. WATER IS THE ONLY DRINK ALLOWED IN THE CLASSROOMS. ANY SNACK WITH PEANUT PRODUCTS IS NOT ALLOWED. CHILDREN ARE NOT ALLOWED TO SHARE FOOD.
LIBRARY • Students in K-2 will come to the library once a week to check out books. Every other week there will be a short library lesson. • The RAZ-Kids Reading Incentive program is used in K-2 to encourage students to read on a daily basis. Students who listen to , read and take a quiz for 150 books will be invited to the Ice Cream Party on May 1 st. BIRTHDAYS Birthday Treats Students may enjoy store bought birthday treats (i. e. cupcakes or cookies) the last 5 minutes of school. The students can eat their treat and then go home. Please inform your child’s teacher before sending treats. Please do not bring birthday invitations to class. If you would like to be on a class birthday list, please fill out our contact form on the computer before you leave.

DRESS CODE • Students are to wear a collared, plain, navy, black, light blue, red, or white shirt. It can have a small logo on it. • Sweatshirts and hoodies must be solid navy, light blue, red, white, or the Hubenak hoodie. Solid denim jackets are allowed. Hats and hoods must be removed in the building. • Students may also wear an approved Hubenak shirt such as the PTA Hubenak T-shirt or the grade level shirt. Friday is our spirit day where children can wear their class T-shirt.
DRESS CODE CONTINUED… Student’s pants or skirts need to be denim, navy, or khaki. Jeggings are not allowed unless worn under a skirt. Athletic or sweat pant material is not an approved item • Shorts or skirts must be no shorter than fingertip length. • Shoes must be safe for recess and physical education and must have backs. Flip-flops, backless shoes, etc. are not allowed. • Extremes in hair styles; mohawks; psychedelic colors; lines, letters or designs shaved in the head is considered inappropriate.

DEVICES CELLULAR PHONES, TELECOMMUNICATION DEVICES, MP 3 PLAYERS, CAMERAS, OR ANY OTHER TYPES OF ELECTRONIC DEVICES ARE NOT ALLOWED TO BE TURNED ON OR IN USE INSIDE THE SCHOOL BUILDING UNLESS BEING USED FOR SPECIFIC INSTRUCTIONAL ACTIVITIES. CAMPUSES HAVE THE DISCRETION TO IDENTIFY APPROPRIATE OPPORTUNITIES FOR ELECTRONIC DEVICE USE. PTA WILL BE TAKING ORDERS FOR A 1 ST GRADE T-SHIRT THAT CAN BE WORN ANY DAY WITH UNIFORM BOTTOMS.
