FABOH Self-Care Program � What: A comprehensive Self-Care intervention offered to FABOH Member companies � Eligible: All FABOH Member companies � Resources provided by FABOH include: �Self-Care books �Customizable Power. Point presentation to train employees on how to use the book and the value it provides �Follow-up quizzes to ensure employee engagement �Program evaluation materials (WELCOA Specific) �WELCOA Personal Health Record document to distribute to participants �Personal support and assistance from FABOH Wellness Director
Program Statistics Since Last Year’s Conference: � 11 member companies participated � 2, 229 Self-care books were distributed to FABOH Members Overall including FABOH Wellness Grants from previous years: � 14 member companies participated � 3, 059 Self-Care books were distributed to FABOH Members
Evaluation Outcomes Includes data from 4 employers going back to surveys collected in March 2013: � 179 surveys returned � 55. 8% reported referring to the book � Average of 3. 43 times per � 28 completed a Personal Health Record (Implemented in 2013) � 95. 3% satisfied or very satisfied with the program � 60 doctor’s visits avoided � Cost avoidance of $11, 220 ($187 per visit) � 10 emergency room visits avoided � Cost avoidance of $11, 260 ($1, 126 per visit) Total Cost Avoidance of $22, 480
Key Take Away Points �The shelf-life of these books is no less than 3 years �To date the program has nearly paid for itself in cost avoidance based on the reporting of only 4 of the 14 employers �Economic return from these books will continue as long as promotion of the books continue and programming remains constant �This program meets the requirements for the Self. Care intervention needed to achieve Well Workplace designation

What is Well City? Well City is an initiative based around WELCOA’s (Wellness Council of America) Well- Workplace award designed to engage entire business communities in improving the health and well-being of their workforce The Vision: To make Fond du Lac’s workforce one of the healthiest in America by developing and delivering results‐oriented, evidence‐based health promotion programs at the worksite
The Goal of Well City Fond du Lac To ensure that a minimum of 20 employers, who collectively employ 20% (~5, 900 employees) of the area’s workforce, achieve Well-Work Place designation within a three year period

National Employee Health & Fitness Day Employer Challenge What: A one week physical activity and healthy behavior challenge offered to Fond du Lac County Employers �A turn-key toolkit was available for all site leaders to help maximize participation �All participants in the program received a 1 -week free trial membership at participating fitness centers within the area �Participants tracked their minutes of strength, cardiovascular and stretching exercises as well as their completion of bonus point activities �Employers logging the most minutes per participant and highest participation percentage within their division were recognized

2013 Get Up & Go Employer Challenge What? � A 3 -month exercise incentive program/Community Challenge (9/9 – 11/22) Who was eligible to participate? � All employers in Fond du Lac County How the challenge worked? � Participating employers used the comprehensive toolkit provided to customize a program layout and incentive strategy which fit their specific objectives and budget � Similar sized employers competed against each other to earn public recognition, a trophy, and most importantly bragging rights until next year’s challenge � Participants logged minutes using the FABOH Exercise Tracker to minimize the resources needed to implement ** Program design and evaluation materials met the requirements needed for the Exercise intervention within both the WELCOA Small Business and Well Workplace applications**

Pre-Diabetes Defining Pre-diabetes: �Pre-diabetes is a serious health condition that increases the risk of developing type 2 diabetes, heart disease, and stroke � Those with pre-diabetes - have blood sugar levels that are higher than normal, but not high enough for a diagnosis of diabetes �Without lifestyle changes to improve their health, 15% to 30% of people with pre-diabetes will develop type 2 diabetes within 5 years
Pre-Diabetes The Magnitude: �Nationwide- 79 million Americans – 35% of those aged 20 or older have pre-diabetes �Statewide- 1. 46 million aged 20 or older have pre-diabetes �It is estimated that Fond du Lac County has 26, 040 pre-diabetics. (2011 Burden of Diabetes in Fond du Lac County) �An estimated 21. 5% of FABOH members are in the pre-diabetic range (Averaged from HRA results of 9 members)
Pre-Diabetes The Cost of Diabetes: � Nationwide (2012 data): �$176 billion for medical costs �$69 billion productivity costs (indirect) �Total Cost: $245 billion � Statewide (2009 data): �$4. 07 billion in direct costs �$2. 04 billion in indirect costs �Total Cost: $6. 1 billion � Fond du Lac County (2009 data): �$79. 3 million in direct costs �$39. 7 million in indirect costs �Total Cost: $119 million
Creating a Successful Strategy at Your Worksite: 1. Raise awareness of pre-diabetes amongst employees � Just 7% of people with pre-diabetes are aware they condition (CDC) � Provide biometric screenings including glucose with HRA’s � Send targeted letters to those with pre-diabetic glucose levels � Launch an educational campaign on the dangers of pre-diabetes 2. Identify successful intervention options � Research shows that modest weight loss (5 -7% of body weight) and regular physical activity can help prevent or delay type 2 diabetes by up to 58 percent in people with pre-diabetes (CDC) � Getting at least 150 minutes each week of physical activity, such as brisk walking, also is important (CDC) � The National Diabetes Prevention Program led by the CDC � Health Coaching � Intervention from health care providers 3. Develop incentives to drive engagement 4. Implement and evaluate
