Greater Manchester VISION • ‘Across GM we aim to deliver the fastest and greatest improvement in the health and wellbeing of our whols population, creating a strong, safe and sustainable health and care system that is fit for the future. ’ • GM ambition to establish fully integrated community based health care provided by MDT who work directly with patients across primary care and hospitals. • GM primary care strategy & workforce plan identified programmes required to support emerging leaders to develop skills and knowledge to lead the GM vision. • This programme is designed to support this vision as a foundation for nurses to develop leadership skills for the future.

CARE programme Bespoke programme of support and development for nurse leadership in general practice and primary care networks across GM Collaboration with system leaders, NAPC, NHSE/I & Shiny Mind a bespoke programme will be accessible for 67 GPN’s across GM
CARE Programme The programme supports participants to connect with the wider system transformation, to influence at all levels and furthermore to empower leadership in action Providing the basis that will help the system build a sustainable long term approach. Anticipated impacts evaluated throughout and at the end Additional resources have been identified to support the programme
