Post-op Risk Stratification • All patients must be re-stratified after surgery • Stratification Tool can be found: • • Post-op Transfer Navigator Admission Navigator
Post-op Risk Stratification � Complete RISK Stratification before using order set � If not stratified, clinician will receive BPA to complete (click on hyperlink and then close and reopen order set to receive correct orders). � If not completed prior to opening order set all options for VTE prophylaxis will be displayed. These will not be specific to your service guidelines.
Post-op Notification for Staff �VTE Status can be found in the Header or Patient List Column
Post-op Notification for Staff �Patient Education Topic will be automatically added for documentation
Discharge for VERY HIGH RISK Patients �Current surgery guidelines include 30 days of prophylaxis for VERY HIGH RISK patients. �Some surgery discharge order sets contain suggestions for discharge medications or standalone mediation orders can be placed as well.
