Consent forms n n Designed to be legal, binding document in veterinary medicine. Allows client to be informed about treatments or procedures. Over 40% of informed consent is not valid. Been demonstrated that only 19% of individuals who signed informed consent had a better understanding after signing form.
Consent form Rules n n Should give accurate and sufficient information regarding procedures, expected outcomes and methods used. A copy of consent form should be given to the client (is the receipt of what is to occur).
Types of Consent Forms n Medical Treatment Consent Form Should include owner and animal information. n Contact number for client. n Primary Complaints. n Consent for treatment (maybe itemize which treatments to perform). n Agreement of payment. n Signature and date. n
Surgical/Anesthetic consent forms n n n n Client and pet information. Phone number. Type of procedure to be performed. Resuscitation efforts owner desires. Pre-anesthetic bloodwork, microchipping, other optional services (add on dentals), etc. Payment agreement. Signature and date.
Off Label/Internet Consent Forms n n n Should clearly state if something is being given off-label. Patient information. “No guarantee” clause.
Euthanasia Consent Forms n n n Client and Pet information. Disposal agreement. “No Bite” clause.

Transportation Waivers n n n Allows staff to transport animal on client’s behalf. Should have client and patient information. “Not hold liable” clause

Pet Sitter Releases n n Releases a clinic from anything done by an employee outside work environment. Allows client to realize that employee is not a representative of clinic while pet sitting. Should give information regarding medical decisions. Signature and date.
