The Purpose of EBP • • • Translate research into practice Increase the effectiveness of treatment Encourage some consistency in practice Utilize interdisciplinary approaches Encourage practice that is not based on tradition
Grading EBP (U. S. Preventive Services Task Force) • Level I: Evidence obtained from at least 1 properly designed randomized controlled trial • Level II-1: Evidence obtained from well designed controlled trials without randomization • Level II-2: Evidence obtained from well designed cohort or case control analytic studies, preferably from more than one center or research group • Level II-3: Evidence obtained from multiple time series with or without the intervention. • Level III: Opinions of respected authorities, based on clinical experience, descriptive studies, or reports of expert committees.
Grading EBP (US Preventive Service Task Force) • Level A: Good scientific evidence suggests that the benefits of the clinical service substantially outweighs the potential risks. Clinicians should discuss the service with eligible patients. • Level B: At least fair scientific evidence suggest that the benefits of the clinical service outweighs the potential risks. Clinicians should discuss the service with eligible patients. • Level C: At least fair scientific evidence suggests that there are benefits provided by the clinical service, but the balance between benefits and risks are too close for making general recommendations. Clinicians need not offer it unless there are individual consideration. • Level D: At least fair scientific evidence suggests that the risks of the clinical service outweighs potential benefits. Clinicians should not routinely offer the service to asymptomatic patients. • Level I: Scientific evidence is lacking, of poor quality, or conflicting, such that the risk versus benefit balance cannot be assessed. Clinicians should help patients understand the uncertainty surrounding the clinical service.

Example of PICOT • Population: Study Geriatric patients with functional deficits • Intervention: Employ Restorative Program once patients have completed KT/PT program • Compare: Are other programs being utilized and how do they compare to a restorative program • Outcome: Was a difference noted or did the intervention (restorative program) enhance the functional ability of the geriatric patient compared to normal treatment • Time: May, 2015 to August, 2015
Steps to EBP • • • Select a topic to be studied Critique current evidence Adapt the evidence into practice (if needed) • Implement the EBP • Evaluate the outcome (either on patient or process)
Research Definition • Systematic inquiry that uses disciplined methods to answer questions or solve problems. • Generates new knowledge or validates knowledge. • Generalizable.
Differences: EBP • Interventions are stricter and more prescriptive than QI protocols but not as strict as research • Data collection may not be rapid and involves varying resources
Differences: Research • Has tight controls for extraneous variables to provide confidence that outcomes occur as a result of defined intervention and not chance • Not rapid and involves the use of resources that may vary according to scope of the study
Quality Improvement Definition • To improve internal processes and practices within a specific patient group or organization.

Case Study 2 What are the best screening techniques for colorectal cancer?

Case Study 3 A patient has a grand mal seizure. He has no prior hx of seizures, and no head trauma. He drank ETOH once or twice a week and had not had ETOH on the day of the seizure. PE is Negative. The pt is given a loading dose of Phenytoin IV and the drug is continued orally. A CT head is normal. EEG shows only nonspecific findings. The pt asks what is his risk of recurrence of seizures.
After a Literature Search on Grateful Med ($2. 68 and one half hour later), the clinical evidence demonstrates the risk of recurrence at one year is between 30 -43%. After a seizure free period of 18 months, the risk of recurrence would be under 20%. The patient now leaves with a clear idea of his prognosis.
