UNCLASSIFIED / FOUO Step 3: Request Forms All personnel complete the attached FORM 102. ―Do not sign the FORM 102. Personnel who do not possess a valid CAC complete the attached FORM 5000. ―Do not sign the Form 5000. ―If you have a valid CAC, DO NOT fill out a FORM 5000 4 UNCLASSIFIED / FOUO

UNCLASSIFIED / FOUO Step 4: Email Notification ― All visitor organizations must send an email to the POC below. JPAS VARs without accompanying email may not be processed in a timely manner. ― Subject title must read “ Appropriate Program Name “ ― Email must be sent by your organization security POC and must include all visitors from your organization in one email (No individual emails will be processed). ― Include a completed Record of Organizational Training on Unexploded Ordnance Range Hazards form for each visitor requesting unescorted uprange access ― Include a statement that all visitors requesting uprange access have read and understand the WSMR Photo Policy ― Include completed Form 102 ― Include completed Form 5000 if required Visit Request POC ― Ralph Gallegos (Security Coordinator) – ralph. r. gallegos. civ@mail. mil, ― 575 -678 - 8106, Fax: 575 -678 -2143 5 UNCLASSIFIED / FOUO

UNCLASSIFIED / FOUO Obtaining Access to Uprange Areas with a CAC 6 Visit requests accompanied by all completed forms will be cleared to receive uprange access. ―Applicant must carry a printed copy of the approved Form 102 and their CAC at all times. • Applicants without a CAC must carry a printed approved FORM 5000 in addition to the approved Form 102. ―Range access will expire at the end of the time requested on the FORM 102 Note: Visitors wishing to view the mission from Range Control and not traveling uprange are required to be on the access list at the front desk. UNCLASSIFIED / FOUO Obtaining Access to Range Control Visitors requesting access to the Range Control facility can do so as part of the visit request process. -Sponsoring Organization will provide an access list to front desk. Upon arrival to the facility, WSMR security personnel will verify visitors identification and an escort will be provided to guide visitors to the viewing areas. Due to the nature of the facility, no electronic devices are permitted beyond the security checkpoint.
