Current provision DESMOND DIABETES EDUCATION and SELF MANAGEMENT for ONGOING and NEWLY DIAGNOSED Who. All newly diagnosed Type 2 DM. Existing Type 2 DM non-insulin treated. Suitable for group education What. 1 day (6 hours) Where. South . GP/PN . Peckwater Centre . Selfreferral to Diabetes Service Camden Centre for Health . 10 Patients. 2 HCPs How referral via CICS
What is DESMOND? • Self-management programme based on theoretical basis Considering; – How people make sense of information to influence their beliefs and behaviors – The impact of social factors on learning, confidence and behavior change – The role of implementing intentions and goal setting in supporting behavior change • Person – centered philosophy

Initial comments from patients My GP said that I am borderline/ I have mild diabetes I do not know why I am here? What can I eat or not eat? I do not know why I got DM I don’t think I have DM yet as my sugars are not high? Can DM be cured? Can I stop medication if my DM is better ?
Feedback after DESMOND What sort of things do you think are important? What do you understand better? “Learning to self-manage” “The interactions about the various types of management” “I now know how serious it is” “That I cannot ignore the warning and that I must be sensible to live longer + better!” “Going for regular tests, portion control, lowering intake of sugar, salt and saturated fats” “I feel good in myself and more relax” “I do not know why more people do not come to DESMOND” “Very helpful-at least I know what food to avoid and what to eat” “What is happening in my body” “Can I come back again? ”
