Konvenční endotracheální odsávání Endotracheální odsávání je nezbytnou procedurou u intubovaných pacientů v kritickém stavu • Intubace dýchacích cest = porušení funkce “mucociliárního eskalátoru” a schopnosti odklašlávání • Endotracheální odsávání umožňuje odstranění sekretů, hlenu a exudátu z respiračního traktu
Konvenční endotracheální odsávání Proč musí být pacient odsáván? Indikace • Sekrety viditelné v ET rource • Bublavé zvuky z respiračního traktu • Increased peak inspiratory pressure or decreased Vt • Náhlý pokles úrovně O 2 • Hrozící aspirace žaludečního obsahu • Po vyfouknutí obturačního balónku Ventilovaní pacienti jsou odsávání většinou v pravidelných intervalech ( 6 - 12 x /den)
Konvenční endotracheální odsávání vyžaduje ODPOJENÍ PACIENTA OD DÝCHACÍHO OKRUHU tak aby bylo možné zasunout odsávací katetr do tracheální/tracheostomické rourky ØPŘERUŠENÍ VENTILACE / APNOE ØEXPOSE DÝHACÍCH CEST VNĚJŠÍMU PROSTŘEDÍ

Ty-Care exel: Instructions for Use How to connect Ty-Care exel to breathing system 1. 2. 3. 1. Remove protective cover, lift protection lid and connect suction control valve to suction line. 2. Set vacuum to desired level while depressing suction control valve. 3. Connect Ty-Care exel to breathing circuit and tracheal/ tracheostomy tube. If needed interpose catheter mount.
Ty-Care exel: Instructions for Use How to perform suctioning 1. 2. 1. Rotate ANTICLOCKWISE the patient access in the open position. 2. Insert suction catheter with one hand into tracheal / tracheostomy tube to desired depth. Maintain grip on fixed connector part with the other hand.
Ty-Care exel: Instructions for Use How to perform suctioning 3. 4. Apply vacuum by depressing suction control valve, whilst withdrawing suction catheter. 4. After suctioning, withdraw suction catheter until BLACK TIP MARKING is fully visible.

Ty-Care exel: Instructions for Use How to clean the suction catheter 1. 2. 1. Ensure patient access valve in CLOSED position to isolate suction catheter from airway. 2. Connect vial or syringe to lavage port and inject solution while applying suction by pressing control valve.
Ty-Care exel: Instructions for Use How to remove suction catheter in closed system 1. 2. 1. Ensure patient access valve is CLOSED position to isolate suction catheter from airway. Rotate suction catheter clockwise and remove it. 2. To protect catheter for possible reuse on the same patient, seal using the sterile cap provided.
Ty-Care exel: Instructions for Use How to perform bronchoscopy or mucus sampling in closed system 1. 2. 1. After removing suction catheter, place self-sealing cap over access port. 2. Turn to OPEN position, insert bronchoscope or mucus sampling catheter and perform procedure. Upon completion of procedure, turn to CLOSED position.
Ty-Care exel: Instructions for Use How to administer aerosol using MDI (Metered Dose Inhaler ) port Remove the cap from MDI port and insert can tip with moderate pressure. Set the canister in vertical position and press it during inspiratory phase. Remove canister and recap port.
