TUITION WAIVERS IN DOCUSIGN – FAMILY MEMBER WAIVER
EMPLOYEE PORTION 1. After clicking the link, Enter your name and email address and your signing family member’s name and email address. 2. Click to accept The terms/conditions.
3. Begin to complete the waiver form. Areas in red are required. Then click “sign” by Employee Signature. Notice the “attachment” at the top of the page. Here you can attach necessary verification documents if you have not already sent those in.
5. Once completed and signed, you can click “FINISH” in the upper right corner. The employee portion is complete. You can print one for your own files if necessary. 4. It will prompt Employee to choose a signature style. Select preferred style, then click “adopt and sign”.
FAMILY MEMBER PORTION 6. Family Member will receive an email from Docu. Sign to review and sign the Waiver Form. Family Member will click “Review Document”. 7. Click to accept The terms/conditions.
8. Family Member now can review the document submitted. Family member can click “sign” in the Required Family Member Approval box.
10. It will prompt Family member to choose a signature style. Select preferred signature, then click “adopt and sign”. 11. Notice the “attachment” at the top of the page. Here you can attach necessary verification documents if you have not already sent those in, or if employee missed this step.
Once family member signs, click “FINISH” in upper right corner to complete waiver process. Family member portion is complete! Form will automatically route to the additional approving departments for signatures.
Employee/Family Member do not need to sign up for Docu. Sign account. You can click “no thanks”. However, it is FREE to create a Docu. Sign account for future signatures!
