Casus: 59 -jarige Antilliaanse man • Voorgeschiedenis: blanco, sinds 15 jaar in NL • Hetero-anamnese via echtgenote: • enkele dagen braken (bijmenging bloed) • geen melaena • hoofdpijn • verward • Med: geen • Intox: af en toe roken en alcohol, geen drugs

Vervolg casus • Lichamelijk onderzoek: • T 37. 9, RR 110/65, pols 76, sat 98%, 80 kg • negroïde man, niet helemaal adequaat • spoor helder bloed in mond • cor: systolische souffle gr 2/6 • RT: geen afwijkingen

Behandeling • Onbehandeld: mortaliteit 90% • Sinds plasma exchange: mortaliteit 10 -30% • Rationale plasma exchange • Dilutie antistoffen • Repletie v. WF-protease

Vervolg casus • Dag 9 • geen reactie op plasmaferese en prednison • 1 e insult (geen aanwijzing bloeding op CT) • Start difantoïne • plasmaferese 4 » 6 liter/dag • start rituximab

Vervolg casus • Dag 13 • Neurologische verslechtering • start ciclosporine 2 dd 100 mg oraal • start haldol • Dag 23 • Refractaire TTP • plasmaferese, prednison, ciclosporine en rituximab • Neurologische verslechtering • Transfusie-behoefte erytrocyten
