Aim: To support a reduction in discharge delays The Trusted Assessor role; • Is an independent and unbiased assessor supporting the discharge process. • Undertakes assessments to support and facilitate effective and timely discharges from Hospital settings into care homes. • Liaises with the care homes about the discharge arrangements to streamline processes. • Acts as a point of contact when residents who are admitted to hospital from a care home to monitor progress and potential return. • Brings together health, social care and providers to enable timely and safe discharges.

Assisting with the discharge process • Liaison – Patient, Ward Staff, Doctors, care home managers, therapy teams, social care • Readiness Checks – Equipment, transport, medications • Adding Value – Recognising where additional assessments may be needed, for example SALT/DOLs – Pre-emptively answer care home queries – Saving wasted journeys for care home managers – Providing accurate assessments of patients’ needs
