Role of Case Management in Treating Depression
Understanding Provider Role in Treating Depression The Case Manager’s (CM) role is to provide routine case management throughout the time the client is in services with the LMHA The tasks involved include: Ø conduct assessments (CANS, Quarterly, and Treatment Plans) in order to evaluate symptoms of the DX and determine level of care Ø facilitating skills sessions teaching a certain curriculum for the specific DX Ø collaborates with both client and LAR (Legally Authorized Representative) to assist in locating community resources Ø teaches medication management in order to maintain compliance of medication needs Ø provides crisis intervention and referrals to local behavioral hospitals when needed
Considerations when Interacting with Clients Ø Understanding your own biases and how they affect your efficacy in treatment Ø Showing empathy when dealing with all clients Ø Using collaboration with other mental health professionals (within confidentiality) Ø Learning your client’s story beforehand (Biopsychosocial) Ø Focus on natural supports (school, family, etc. ) Ø Being culturally aware of your client and their family dynamics Ø Using a strength based approach for treatment
Our Treatment Plan Healthcare Domain: Behavioral Health Provider: Case Manager Goals: Objective: Decrease in symptoms as indicated by client report of: (a) Increase in social interaction/participation in events (b) Improvement in grades (c) Less suicidal thoughts (d) Increased communication with his father a) Andrew will have interest in life again b) Andrew will have a hobby he enjoys c) Andrew will have a relationship with his father Interventions: a) Identify ways to participate in school without football as well as finding new hobbies b) Learning and implementing effective communication skills in order to express and understand his feelings and others c)Identify strategies to cope when feeling depressed such as relaxation and unhelpful thinking etc.
Measuring Outcomes/Success • Self report on client’s social interactions increasing to at least 1 outing a week with a friend/family • Improvement on grades determined by self reporting and assessment by teachers • Self report on how many times client had a positive interaction with his father during the week • Suicide assessment self report by client weekly to identify decrease in suicide symptoms
Communication Needed from Counselors • Staffing regularly between case manager and counselors about client interactions, family changes, crisis situations etc. allows for the team to better assist the client in treatment • Counselors are able to provide their diagnostic assessment of client’s needs which assists the CM in creating the treatment plan and providing additional services
