Defining Traumatic Brain Injury § Traumatic brain injury (TBI) is a [mechanically] induced disruption of brain functioning, from the head being struck [Acceleration; assault], head striking an object [Deceleration; fall], or the brain undergoing acceleration/deceleration movement w/o external trauma to the head [Pure Impulse Injury; whiplash] and characterized by: Ø Any period of loss of consciousness Ø Any anterograde or retrograde amnesia Ø Any alteration of mental state at the time of the accident (e. g. feeling dazed, disoriented, or confused) Adapted from The American Congress of Rehabilitation Medicine, 1993 § An insult to the skull, brain, or its covering, resulting from external trauma that produces an altered state of consciousness or anatomic motor, sensory, cognitive, or behavioral deficits. Classification of Evidence (Prognosis) • Class I Evidence: Prospective, randomized, controlled trials (PRCT) – the gold standard of clinical trials. However, some may be poorly designed, lack sufficient patient numbers, or suffer from other methodological inadequacies. • Class II Evidence: Clinical studies in which the data were collected prospectively, and retrospective analyses were based on clearly reliable data. Types of studies so classified include: Observational studies, cohort studies, prevalence studies and casecontrolled studies. • Class III Evidence: Most studies based on retrospectively collected data. Evidence used in this class indicates clinical series, databases or registries, case reviews, case reports, and expert opinion with some support from animal studies.

Cognitive Rehabilitation • Any systematic program directed at modifying cognitive function or cognitively oriented activities following acquired brain injury. • Direct Retraining Approach: Repeated engagement in therapeutic exercises and tasks that stress disrupted cognitive systems will eventually enhance or restore the dysfunctional component. • Functional Compensation Approach: Involves the use of any and all strategies, techniques, devices, and adaptive equipment available to increase functional performance and enable to patient to perform tasks that can no longer be performed in the conventional manner. Functional Compensation Strategies for Impaired Attention 1. Get enough sleep as sleep-loss impairs attention. Take a brief nap in the afternoon if necessary. 2. Determine what times during the day you are most attentive and complete mentally demanding tasks at these times. 3. Arrange your work environment to minimize auditory (e. g. , go to quiet room) and visual (e. g. , work facing a wall rather than a window, close curtains) distractions. 4. Use headphones or earplugs to reduce environmental distractions. 5. Avoid multi-tasking. Work on one project at a time and put it away before beginning another. 6.
