CLIENT HISTORY REVIEW (CHR) A document generated by diagnostics in the community, it provides information in the following areas: Critical information, name, DOB, offense, case number, minimum-maximum, ISCA score, county of residence, ethnicity, time served Length of sentence and committing offense description Suicide and Self Harm History Mental Health History Assaultive/Aggressive History Escape History Detention Behaviors Chemical Dependency Sex Offense Information Education and Vocational Information Medical Concerns Family Information Community Support and Resources Placement Information
Team Exercise Each team will receive a client history review. Use the information to develop a transition plan for each specific case. You will have 45 minutes to identify how your client will transition through the system. Address specific information in each area of need. Your team will provide a 10 minute presentation on the Transition Plan you have for the identified for youth.
