Select the “My Claims and Costs” Tab….
… and then select “Request Claims and Premium Reports” from the drop down menu.
You will have to request the report(s) you want. In this case, “Premium Rate Statements”
Select your account number…. When the next year Premium Rate Statements become available, please remember to forward those off to us as well so we can keep your file current. …and then select the year you want. At a minimum the City will want the current year and the previous two years as well.
Within 24 hours you will have copies emailed to you and they will be now available through your my. WCB account.
