Why the United States preterm birth rate is declining The preterm birth rate in the US declined to 11. 4% in 2013, the lowest level since 1997. Reasons: • A reduced teenage birth rate • Fewer higher-order multiple births • A public policy shift to prevent nonmedically indicated births at < 39 weeks’ gestation • Smoking bans in several stages • Interventions such as progesterone supplement, and the use of cerclage in selected populations Schoen CN, et al. Progesterone and preterm birth prevention
“Progesterone deficient state” has been proposed to be a mechanism of disease in preterm labor
Proposed mechanisms of action reported for progesterone to prevent preterm birth • Stimulate transcription of ZEB 1 and ZEB 2, which inhibit connexin 43 (gap-junction protein that helps synchronize contractile activity) and oxytocin-receptor gene • Decrease prostaglandin synthesis, infection-mediated cytokine production (antiinflammatory effects) by fetal membranes/placenta • Changes in PR-A and PR-B expression (decreased PR-A/PR-B ratio keeps uterus quiescent) • Membrane-bound PR in myometrium • PRs, when stimulated by progesterone, help selected gene promotion, or prevent binding of other factors • Interfere with cortisol-mediated regulation of placental gene expression • Nongenomic pathways • Reduce cervical stromal degradation in cervix • Alter barrier to ascending inflammation/infection in cervix • Reduce contraction frequency in myometrium • Attenuate response to hemorrhage/inflammation in decidua • Alter estrogen synthesis in fetal membranes/placenta • Alter fetal endocrine-mediated effects

Strategy in the prevention of PTB (2016) In twin pregnancies with a short cervical length: • Vaginal progesterone may be effective in the reduction of adverse perinatal outcomes. • RCT is needed to confirm this hypothesis. • Does not reduce the rate of PTB in unselected twin cohorts. • Should not be used in the prevention of PTB in twin pregnancy. Cervical pessary could be beneficial in pregnant women with a short cervix carrying twins.

Cost-effectiveness and decision analyses • Combination of universal TVS CL screening and vaginal progesterone administration to women with a short cervix is a cost-effective intervention that prevents PTB and associated perinatal morbidity and mortality. • Universal TVS CL and treatment with vaginal progesterone for singleton gestations in the US would result in an annual reduction of approximately 30, 000 PTB before GA 34 weeks and of 17, 500 cases of major neonatal morbidity or neonatal mortality. Nifedipine as a tocolytic agent today • The optimal initial dose appears to be 10 mg orally or sublingually. • If contractions persist, this dose could be repeated every 15 -20 min up to a maximal total dose of 40 mg during the first hr of treatment and then 20 mg orally every 6 -8 hr for 2 -3 d. • Side effects: hypotension, tachycardia, palpitations, flushing, headaches, dizziness, nausea
The U. S. FDA is warning that terbutaline administered by injection or through an infusion pump should not be used in pregnant women for prevention or prolonged )beyond 48 -72 hours (treatment of preterm labor due to the potential for serious maternal heart problems and death. In addition, oral terbutaline tablets should not be used for prevention or treatment of preterm labor.
Should women with preterm contractions but without cervical change be treated?
Preterm contraction without cervical change • Regular preterm contractions are common; however, these contractions do not reliably predict which women will have subsequent progressive cervical change. • In a study of 763 women who had unscheduled triage visits for symptoms of preterm labor, only 18% gave birth before GA 37 wk and only 3% gave birth within 2 wk of presenting with symptoms. • No evidence exists to support the use of prophylactic tocolytic therapy, home uterine activity monitoring, cerclage, or narcotics to prevent preterm delivery in women with contractions but no cervical change. • Conclusions: Women with preterm contractions without cervical change, especially those with a cervical dilation of less than 2 cm, generally should not be treated with tocolytics.
Should tocolytics be used after acute therapy?
Maintenance therapy with tocolytics is ineffective for preventing preterm birth and improving neonatal outcomes and is not recommended for this purpose.

Antenatal steroid therapy for fetal maturation • A single repeat course of antenatal corticosteroids should be considered in women who are less than 340/7 weeks of gestation who have an imminent risk of preterm delivery within the next 7 days, and whose prior course of antenatal corticosteroids was administered more than 14 days previously. • Rescue course corticosteroids could be provided as early as 7 days from the prior dose, if indicated by the clinical scenario.
Antenatal steroid therapy for fetal maturation • Betamethasone (12 mg) given intramuscularly 24 hr apart for 2 doses • Dexamethasone (6 mg) given intramuscularly every 12 hr for 4 doses
Is there a role for antibiotics in preterm labor?
Antibiotics should not be used to prolong gestation or improve neonatal outcomes in women with preterm labor and intact membranes. This recommendation is distinct from recommendations for antibiotic use for preterm PROM and group B streptococci carrier status

Is there a role for nonpharmacologic management of women with preterm contractions or preterm labor?
• Bed rest , sedation, and hydration have been recommended to women with symptoms of preterm labor to prevent preterm delivery, these measures have not been shown to be effective for the prevention of preterm birth and should not be routinely recommended. • The potential harm, including venous thromboembolism, bone demineralization, and deconditioning, and the negative effects, such as loss of employment, should not be underestimated.
Preterm labor and multiple gestations • Tocolytics to inhibit labor: greater risk of maternal complications • Prophylactic tocolytics: not reduce the risk of preterm birth or improve neonatal outcomes • Antenatal corticosteroids (magnesium sulfate for fetal neuroprotection): recommended
Timing of cord clamping for preterm babies (born vaginally or by C/S) • If a preterm baby needs to be moved away from the mother for resuscitation, or there is significant maternal bleeding: ▫ consider milking the cord and ▫ clamp the cord as soon as possible. • Wait at least 30 sec, but no longer than 3 min, before clamping the cord of preterm babies if the mother and baby are stable. • Position the baby at or below the level of the placenta before clamping the cord.
