MIPPA Grant • Medicare Beneficiary Outreach and Assistance Program (MIPPA) will provide valuable support to eligible Native American elders for the Low Income Subsidy program, Medicare Savings Program, Medicare Part D, Medicare prevention benefits and screenings, and in assisting beneficiaries in applying for benefits. • The grants are used to coordinate at least one community announcement and at least one community outreach event to inform and assist eligible American Indian, Alaska Native or Native Hawaiian elders about the benefits available to them through Medicare Part D, the Low Income Subsidy, the Medicare Savings Program or Medicare prevention benefits and screenings, and counsel those who are eligible. 2
Required Services 1. One community announcement: e. g. , newsletter, public service announcement, online announcement on the Tribal website, a flyer or poster, some targeted mailing, special placemats, etc. , and 2. At least one outreach event about the benefits available through Low Income Subsidy and/or the Medicare Savings Programs: e. g. , a presentation, either in person or through web/video conference, a booth or exhibit at a health fair, or a dedicated enrollment event with partners. 3

Award Information Only current Title VI tribal grantee can apply. Total budget: $270, 000. Duration of budget: 12 months. Anticipated award date is on or before September 30, 2017. Each Title VI tribal applicant will receive at least $1, 000. ACL reserves the right to adjust funding levels subject to number of applicants and availability of funds. • All expenditures must be properly documented and allowable under the terms and conditions of the grant award. • Cost sharing or matching is not required. • • • 5

Application Process • No letter of intent is required. • Complete and sign/date a one-page Application for FY 17 Tribal MIPPA Funding • Date of submission: no later than 11: 59 a. m. Eastern Time on Tuesday, August 25, 2017 – no exception! • Tribal grantees are recommended to submit the application well in advance of the date and time specified above. 7

MIPPA Reports • Narrative report and SF 425 s are due within 90 days of occurrence. • Narrative report should contain a brief description of: 1. the event 2. date 3. location 4. number of participants (no list of names, please) 5. activities including speakers, flyers, and/or handouts • All funds must be obligated no later than the end of the one year project period. Funds must be drawn down from the Payment Management System (PMS) within 90 days following the event. • Narrative and financial status reports can be sent as email attachment to MIPPA. Grants@acl. hhs. gov and also include ACL regional staff in your email. 10

Reminder: Reports for 2016 MIPPA Grant • A Federal Financial Report (SF-425) and a narrative report are both due within 90 days following the outreach event and no later than December 28, 2017. • All funds must be obligated by September 29, 2017 and liquidated by December 28, 2017. • When submitting the narrative report, please include the words “MIPPA Narrative Report”, your organization name, grant number, a description of the event including date, location, number of participants, and activities (e. g. speakers, handouts, etc. ), date of submission, and an authorized signature. You are encouraged to send both reports to MIPPA. Grants@acl. hhs. gov with subject line “MIPPA [FFR or Narrative] Report for [State] [your tribal organization]”. • Check with your tribal financial office or ACL regional office to make sure funds are drawn from Payment Management System (PMS) and Federal Cash Transaction Report (FCTR) is submitted to PMS timely. 12
