Program Eligibility • Enrolled in UPlan medical plan = automatically enrolled in the wellbeing program • 100% voluntary to participate • Spouses on UPlan may participate in all programs the employee can participate in
UPlan Coverage and Points October 1, 2018 -August 31, 2019 Need to Earn: Money Saved in 2020? Wellbeing My Way Pledge Participate in the following up to five times (may be same category each time): ü Volunteer for non-profit ü Self-directed or department led challenge ü Community activities (recreational teams, dance lessons) ü Race (5 k, marathon) ü Learning and development opportunity 25 points per activity up to 125 points total
Tracking and Challenges • Red. Brick Track™ (manual log or sync a device) • Challenges (2 per year) – 1 point per day for a total of 150 points • Bike commuter (September 1, 2018 – August 31, (doubled from last year) • Fit Choices gym reimbursement (8/1/ 2018 - 07/31/2019) – 8 x per month for 6 non-consecutive months for 150 points (up 50 points) – 12 x per month for 6 non-consecutive months for 200 points (up 50 points) 2019) – 25 Rides for 50 points (NEW lower level) – 50 Rides for 150 total points (up 50 points) – 100 Rides for 200 total points (up 50 points)
• Lyft could not provide pricing of memberships for 2018 -2019 program year = UMN could not commit to subsidize without contract • Nice Rides will still count for points for the 2018 -2019 program year

Reimbursement Overview • For programs that require an upfront payment, employees receive 50% reimbursement of the cost, up to $100 back if all program requirements are met • Employee’s and spouse’s incentives are also taxable and dispersed through the employee’s paycheck • Fit Choices will be reflected on the W 2 Form and will impact employee’s taxable income. UMN is covering the FICA tax.

Bakken Center for Spirituality and Healing • One mindfulness class per year University Cooking Classes Twin Cities, Duluth, Morris • One cooking class per year • Two sessions per year (must complete 14 in person meetings within four months) • $20 per month reimbursement or reduction of membership fees if attend 8 times per month (2 per household on UPlan over the age of 18. This means you could save up to $480 per year) Weight Watchers® on-site meetings Fit Choices (Gym reimbursement)
Recruiting Wellbeing Champions • Promote the Wellbeing Program including challenges and events • Collaborate and discuss ways to enhance the program • Provide leadership within your department and serve as a role model for making healthy lifestyle choices • Encourage a healthy work environment and healthy work practices • Attend a half-day retreat a year • Time commitment: average of 1 -3 hours max per month promoting and sharing information Interested?
