Fact Finding…NOT Fault Finding n Purpose of Conducting Accident Investigations is to: n Identify the Root Cause & Implement Corrective Action to Prevent a Reoccurrence n Accidents are Investigated by the Supervisor with the IIPP Site Administrator and others as needed n Reports are Reviewed by the H&SC n OSHA Requires that All Managers & Supervisors be Trained in Accident Investigation Techniques & Town. Specific Procedures.

Cal/OSHA Minimum Documentation Requirements An Inspection is Conducted After Each Accident that Identifies: n Who Completed the Inspection n Unsafe Condition or Practice Identified n Corrective Action Taken Our Goal is to: Find the Root Cause & Correct the Condition
