Opportunities for State Policy Makers Improve Access to Adult Vaccines Vaccine Financing in the United States – Medicare • Federal health insurance for those age 65 years and older, disability, permanent kidney failure • Part B Coverage (named by statute) – influenza, pneumococcal, hepatitis B (for high risk), tetanus for wound care • Part D Coverage (pharmacy platform) – All other vaccines (e. g. Zoster) – Tranx. Rx (interface between pharmacy and medical benefit claims) – Potential cost-share for Part D products (widely varies based on benefit plan) – Medicaid • Adults with coverage through Medicaid expansion have coverage of all ACIP-recommended vaccines, without cost-sharing • States can choose whether to provide coverage of vaccines to adults covered through traditional Medicaid (pre-expansion). All states cover at least one vaccine. NATIONAL VACCINE OFFICE • States reimburse. THEproviders for. PROGRAM administration fee and vaccines, at rates set by the state.

Standards for Adult Immunization Practice • Developed in 1990 to improve vaccine delivery to adults, most recently updated in 2014 by National Vaccine Advisory Committee • All HCPs, including those who do not provide vaccine services, have role in ensuring patients up-to-date on vaccines • Based on key impact of provider recommendation for patient vaccination • Call to action for HCPs for adults to: • ASSESS vaccination status of all patients at every clinical encounter • Strongly RECOMMEND vaccines that patients need • ADMINISTER needed vaccines or REFER to a vaccine service provider • DOCUMENT vaccines received by patients in state vaccine registries Source: Public Health Reports 2014; 129: 115– 123 THE NATIONAL VACCINE PROGRAM OFFICE

The Importance of Adult Vaccination As the U. S. population ages, the impact of vaccine-preventable diseases and their complications among adults is likely to grow • Adult vaccination reduces morbidity and mortality. – – – • 40, 000 cases and 4, 000 deaths attributable to invasive Pneumococcal Disease 3, 000 -49, 000 deaths due to Seasonal Influenza 9, 000 cases of Pertussis (Whooping cough) 3, 000 reported cases of acute Hepatitis B 1, 000 cases of Herpes Zoster (Shingles) Adult vaccination reduces health care costs – Up to $87 billion estimated in annual health and productivity costs of flu alone • Adult vaccination protects families and entire communities – Unvaccinated adults may unknowingly spread vaccine-preventable diseases to people who cannot be vaccinated like small children or people with weakened immune systems like cancer patients, organ transplant recipients, etc. THE NATIONAL VACCINE PROGRAM OFFICE
