Date: Schedule of Visit: 8 May 2004 (Saturday) 9: 15 am ~ Assemble at the Main Entrance facing Waterloo Rd, Kwong Wah Hospital, Yaumatei, Kowloon, HK 9: 30 am to 10: 00 am ~ Visit to SARS Ward Cost: $100 per member/$50 per student member Note: A. All visitors MUST strictly observe the following rules during the visit: 1. . Names and companies of all visitors have to be provided to us before the visit. 3. Temperature check will be conducted before they are allowed to enter the hospital. 4. No recording or photo taking in any means will be allowed. 5. All visitors have to follow the instructions and guidance of HA. KWH staff strictly during the visit. 6. The visit shall only be confined to the wards designated for the visit. 7. The visit shall not last for more than 1 hour. 8. Rights of all patients shall be duly observed. 9. No transportation will be arranged. 10. No parking will be allowed. 1. By signing the reservation form, the visitor accepts that HKIFM does not have any insurance cover for this activity and expressly waives all his/her claims or damages whatsoever howsoever caused against HKIFM for this activity. 2. All visitors are strongly advised to take out an insurance policy to cover this activity and the visitors understand accept that they engage with this activity at their sole risks.
